Please note: The application follows the application instructions below. This form will NOT save. Do not start the form if you will not be able to complete it once you begin as your information will not save until you hit the submit button. You will receive a confirmation note upon hitting the Submit button to verify your form went through.

Please follow the following steps as you complete our application process:

- Considering PCC document: Read through the document to help you determine if PCC would be a good fit for your family
- Doctrine Statement: Read and sign it if you are in agreement
- Member/Student pledges: Read through and sign our member pledge and have your children sign the student pledge
- Course Offerings Chart: Use this chart while you fill out the application for your facilitator role and your student's classes
- Coordinator Role Chart: Use this chart while you fill out your desired coordinator role on the application
- PCC 2019-2020 Operating Calendar: for your reference

2. Application: Fill out the application and submit (Deadline for new prospective members is Friday, February 22 at 9 pm)
3. Mail: the following in one envelope (address will be provided in an email after you submit your application - must be received by Friday, February 22 at 9pm)
4. Reference/Recommendation Form: Ask two people to fill out a reference/recommendation for you. Preferably, one from a pastor, Bible study/small group leader, someone you serve in ministry with and one from a person outside of your family. A short online form has been provided under the PCC Information tab above. This shouldn't take more than 5 minutes for someone to fill out and submit. (Deadline for submission is February 25.)
5. Response: Once everything is submitted: you should receive an email around the second week of March letting you know if we are able to place your family or if we need to put you on our wait list.

- registration fee made out to Colonial Baptist Church in the amount of $220 (registration fee and first child's seed money)
- signed Doctrine Statement
- signed Member Pledge and signed Student Pledge
